About Us
At Active Pathways, we operate as a dedicated specialist health and social care provider specialising in mental health rehabilitation, recovery-focused care pathways, and supported living services for adults with complex mental health needs and personality disorders. Established with a core commitment to empowering independence and progressive recovery, our mission is built on providing safe, therapeutic environments where individuals can rebuild their confidence, develop vital life skills, and successfully transition back into the community.
Who We Are
Operating through specialised residential and rehabilitation units across Lancashire (including facilities such as New Dawn and Ashton House), our organisation brings together experienced consultant psychiatrists, psychiatric nurses, occupational therapists, social workers, and dedicated recovery support teams. Fully regulated by the Care Quality Commission (CQC), we collaborate closely with NHS mental health trusts, Integrated Care Boards (ICBs), and local authorities to deliver structured, evidence-based care pathways.
What We Do
We deliver an integrated suite of specialist mental health rehabilitation and support solutions:
- Mental Health Rehabilitation: Providing structured, medium-to-long-term recovery placements designed to help individuals with severe and enduring mental health conditions manage their symptoms and regain independence.
- Complex Needs & Personality Disorder Support: Delivering specialised therapeutic interventions, psychological support, and behavioural management frameworks tailored to complex presentations.
- Supported Living & Step-Down Services: Offering transitional housing and community-based support designed to bridge the gap between institutional care and fully independent living.
- Occupational Therapy & Skill Building: Engaging residents in structured daily living programs, vocational training, educational pursuits, and social integration activities to promote long-term wellness.
Our Approach & Values
- Recovery-Centred Pathway: Focusing on individual potential, personal choice, and incremental milestones rather than just symptom management.
- Multi-Disciplinary Expertise: Integrating clinical psychology, nursing, and occupational therapy into a unified, holistic support network for every resident.
- Safe Therapeutic Environment: Maintaining secure, welcoming, and well-resourced facilities that balance rigorous clinical safety with a warm, home-like atmosphere.
