This post is to support the Hospital at Home service, which is a collaboration with the acute trust to deliver hospital-standard treatment within a patient’s home. The Hospital at Home philosophy is providing acute clinical care at home, which would otherwise be provided in a hospital.
This service is part of the Urgent Care Services, which provides a two-hour response to patients in crisis at home. Our aim is to prevent unnecessary hospital admissions and facilitate early discharge from the acute hospitals. We also provide unscheduled care out-of-hours for patients who may be at the end of life or experiencing an urgent need for healthcare. This may include treating acutely unwell people who would otherwise require hospital admission, and also supports hospital discharges. The service is available 24 hours a day, 365 days a year.
Nurses use a wide clinical skills base to provide hospital-level care at home. This includes IV therapy, end-of-life symptom control and management via syringe driver, catheterisation skills, phlebotomy, wound management, risk assessment, as well as general observations and fundamental nursing care.
