Overview
The Home First Team Manager is responsible for the effective day-to-day operation of the Home First service.
This includes leading a multidisciplinary team of assessors, community support workers, and administrative staff to ensure the safe and timely delivery of person-centred care that supports recovery and promotes independence at home.

Duties
- Oversee the planning and coordination of staff rotas, ensuring appropriate deployment of support workers and assessors across the locality to meet service demands.
- Monitor service delivery to ensure it aligns with Home priorities and complies with CQC standards, organisational procedures, and national regulatory framework
- Hold responsibility for the management of a defined caseload, ensuring timely assessments, support planning, and discharge coordination.
- Provide direct line management to Band 5 and Band 4 Home First Assessors, community-based support workers, and admin staff, ensuring they are well-supported, supervised, and developed in line with service expectations.
