An exciting opportunity has arisen for a dynamic Nurse or Nurse(s) to join the amazing Poole Intermediate Care Service. We are a multidisciplinary Team who prevent hospital admissions and secures early hospital discharges.
We deliver a hospital-at-home service, caring for patients in the comfort of their own homes.
This is a temporary post to cover maternity leave. The initial contract is for 5 months. However, due to changes within the team, it is very likely to be extended. This can be a secondment or a fixed-term contract. If you are interested, it will be worth your while to ring for more information.
The post will be a great opportunity to develop new skills and experience working in a Hospital at Home Service, and may lead to a permanent post in the future.

Duties
- To demonstrate leadership.
- To provide a high standard of holistic assessment and appropriate, timely reviews.
- To plan specialist care and/or rehabilitation programme, to promote and sustain independence/well-being for the individual in their own homes.
- To deliver Nursing/Rehabilitation/care programmes for adults, many of whom have complex clinical and social needs, to improve independence, function and quality of life.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
This is a fixed-term role, and for those with current NHS service, the role will be offered on a secondment basis only. Please obtain permission to be released on a secondment before submitting an application.
