Who We Are

We are a staff-owned organisation, which means our employees have a direct say in how we operate and how we improve the services we provide. We work in close partnership with the NHS and local authorities to ensure our services support the wider health and care system.

What We Do

We provide an extensive portfolio of community health services, ranging from preventative care to complex long-term condition management:

• Children and Young People’s Services: We offer health visiting, school nursing, speech and language therapy, and specialist support for children with developmental needs or disabilities.
• Adult Community Services: Our teams provide district nursing, podiatry, rehabilitation, and support for those living with long-term conditions like diabetes or respiratory illnesses.
• Urgent Care & Rapid Response: We manage services designed to prevent unnecessary hospital admissions, providing urgent care and support to help people manage health crises in their own homes.
• Specialist Care Pathways: We deliver expert-led care for complex health needs, including specialist therapy services, continence care, and community-based mental health support.

Industry Role

We operate as a Community Interest Company (CIC) and NHS Health Provider. Our role is to act as the primary provider of community health services in our region, filling the gap between primary care (GPs) and secondary care (hospitals). By focusing on community-based treatment, we help keep people healthy at home and reduce the overall burden on the acute hospital system.
