What We Do
We deliver an integrated suite of community care, health, and social support services:
- Shared Lives & Adult Placement: Pioneering the Shared Lives scheme, which matches adults who need support with vetted, welcoming carer households where they can live or stay for short breaks.
- Mental Health & Wellbeing Services: Operating safe spaces, peer support networks, and community-based interventions designed to help individuals navigate mental health recovery.
- Domiciliary & Independent Living Care: Providing tailored practical and personal support that enables older adults and disabled individuals to manage daily life safely in their own homes.
- Reablement & Hospital Discharge Support: Assisting individuals with smooth transitions from hospital to home, rebuilding physical confidence and life skills.
- Family & Carer Support: Providing specialised services and respite opportunities to support unpaid family caregivers and protect their wellbeing.
