We're looking for dedicated Reablement Care Workers to work across Portsmouth and surrounding postcodes, including PO1-PO14, PO16, SO31-SO33, GU31-GU32, through SENDhelp.

What is Reablement?
Reablement is a short-term, intensive care service designed to help people regain skills and confidence after a hospital stay, illness, or decline in health. Rather than doing things for people, reablement focuses on doing things with them - supporting independence, mobility, and daily living skills so they can thrive in their own homes.
The Role - Reablement Care Worker (Portsmouth Area)
You'll be visiting service users in their homes, helping them safely regain independence by:
- Encouraging mobility, personal care, and safe use of the home environment
- Supporting meal prep, medication prompts, and daily routines
- Promoting confidence through goal-setting and progress tracking
- Empowering service users to reduce reliance on long-term care
Areas Covered
You'll be supporting service users across the following postcode areas:
PO1, PO2, PO3, PO4, PO5, PO6, PO7, PO8, PO9, PO10, PO11, PO12, PO13, PO14, PO16, SO31, SO32, SO33, GU31, GU32
Own transport is essential due to the mobile nature of the role.
Requirements
- In-date Care Certificate
- Experience in domiciliary care, hospital discharge teams, or community-based care
- Full UK driving licence + own car
- Ability to work flexibly and independently
- Passion for helping people regain control of their lives
