Care Co-ordinator
Location: Cheshire West & Flintshire

Salary: £27,000
We are recruiting on behalf of a well-established, award-winning home care provider rated Outstanding by the CQC and Excellent by CIW, who are looking to welcome an experienced Care Coordinator into their friendly and supportive office team.
Due to continued growth, this is an additional role within the existing Care Coordination team, offering the opportunity to join a service that is highly respected and genuinely committed to delivering high quality, person-centred care within the community.
They are now seeking someone who has previously worked in a Care Coordinator role and understands the pace, organisation and attention to detail required to successfully manage care delivery in a domiciliary care setting.
The Role
Working closely with the Registered Manager and wider office team, you will be responsible for coordinating care services to ensure customers receive safe, consistent and high-quality support in their own homes.
This is a busy and rewarding role that requires strong scheduling experience, excellent communication skills and the ability to respond effectively to changes within care packages.
Key Responsibilities
- Create and maintain staff rotas to meet customer care needs
- Coordinate care visits in line with customer preferences and staff availability
- Respond to changes in care packages and staffing requirements
- Liaise with care staff regarding availability, holidays and sickness
- Support onboarding of new care staff into existing schedules
- Ensure continuity of care wherever possible
- Maintain accurate electronic records and scheduling systems
- Work collaboratively with the office team to support service delivery
- Communicate effectively with customers and their families where required
About You
- Previous experience working as a Care Coordinator within domiciliary care is essential
- Strong rota management and scheduling experience
- Excellent organisational and time management skills
- Confident communicator, both written and verbal
- Ability to manage a fast paced workload
- High attention to detail
- Experience using care scheduling systems would be advantageous
