Overview
Integrated care is about providing better care and support to our patients, users and the people of Heywood, Middleton and Rochdale by coordinating our services more effectively, with the focus on improving the health and wellbeing of residents.
You will join our multidisciplinary Community Rehabilitation and Falls Team, carrying out holistic assessments of patient needs and developing, implementing, and evaluating treatment programmes.
There is a varied caseload to support adults and older people within their own homes, including care homes.
An opportunity has arisen within the HMR Community Rehab and Falls team for a band 6 physiotherapist.
Duties
- You will work closely with the other team members, who will support your development. You will have experience working within a community setting and help with the delivery of an efficient and effective service.
- You will be an autonomous practitioner and work as a key member of the multidisciplinary team, supervising junior staff.
- You will have an active role in coordinating new referrals, managing waiting lists and supporting the team to maintain patient flow.
- The team works with a broad range of conditions, including medical, surgical, neurological, rehab and mental health conditions.
- The service is community-based and would require you to be able to travel to various locations across HMR on a daily basis.
