Care Coordinator – Domiciliary Care

Contract: Permanent, Full-Time
Pay: £26,500 per annum
About the Role
We are excited to present a fantastic opportunity for an experienced and compassionate **Care Coordinator** to join our well-established domiciliary care service based in the heart of Malvern, Worcestershire. As a Care Coordinator, you will play a pivotal role in the day-to-day coordination and scheduling of care services, ensuring that our clients receive the very best standard of care while supporting and guiding a dedicated team of care workers. This is a rewarding role that truly makes a difference to the lives of vulnerable individuals and their families.
Key Responsibilities
As a Care Coordinator, your responsibilities will include, but are not limited to:
- Coordinating and scheduling care worker rotas to ensure full care coverage across the service at all times
- Acting as the first point of contact for clients, their families, and care workers regarding any queries, concerns, or changes to care plans
- Conducting initial client assessments and regularly reviewing and updating individual care plans in line with changing needs
- Monitoring the quality and delivery of care services to ensure compliance with CQC regulations and company policies
- Supporting the recruitment, induction, and ongoing training and development of care workers
- Managing and responding to any incidents, complaints, or safeguarding concerns in a timely and professional manner
- Building and maintaining strong, trusting relationships with clients, families, healthcare professionals, and external agencies
- Completing accurate and detailed administrative tasks, including maintaining up-to-date client and staff records
- Participating in on-call duties as required to support the smooth running of the service
- Assisting the Registered Manager in ensuring the service meets all regulatory and contractual requirements
Requirements
The ideal candidate will demonstrate the following skills, qualifications, and experience:
- Previous experience working within a domiciliary care or home care setting is highly desirable
- Strong organisational and time management skills with the ability to prioritise a demanding workload
- Excellent communication and interpersonal skills, both written and verbal
- Proficiency in care rostering and scheduling software or a willingness to learn new systems
- A good understanding of CQC regulations and compliance standards within the care sector
- Knowledge of safeguarding procedures and a commitment to protecting vulnerable adults
- A compassionate, empathetic, and person-centred approach to care coordination
- Ability to work effectively both independently and as part of a wider team
- Flexibility to participate in an on-call rota, including evenings and weekends where required
- A valid UK driving licence and access to a reliable vehicle is preferred
- NVQ Level 3 in Health and Social Care or equivalent qualification is advantageous
