We are looking for compassionate and reliable Community Carers to join our team. The successful candidate will support clients in their own homes and within the community, assisting with daily living activities while promoting independence, confidence, and quality of life.
You will provide person-centred care tailored to each individual’s needs, respecting their preferences, routines, culture, and personal choices.
Key Responsibilities
- Provide high-quality care and support to clients in their own homes.
- Assist with personal care, including washing, dressing, grooming, and continence support where required.
- Support clients with medication prompts and administration in line with training and company procedures.
- Prepare meals and support clients with shopping and household tasks.
- Provide companionship and emotional support to reduce loneliness and promote wellbeing.
- Assist clients to access community activities, appointments, and social opportunities.
- Encourage independence and support clients to maintain their skills and confidence.
- Follow individual care plans and accurately record care provided.
- Report any changes in a client’s health, wellbeing, or circumstances to the appropriate person.
- Maintain confidentiality and uphold clients’ dignity, privacy, and rights.
- Work effectively with colleagues, families, and healthcare professionals.
