The postholder will support the registered nursing team in providing care and support to our patients - this could be on the Inpatient Unit at our Hospice, or out in the community with our Care at Home team, visiting patients in their own homes.
This role offers excellent opportunity for you to develop and enhance your healthcare skills and experience within palliative and end of life care.

About The Role - Inpatient Unit:
You will deliver high quality Specialist Palliative and End of Life care as part of the Inpatient Unit multidisciplinary team, under the direction of the designated nursing team.
You will support the assessment, planning and evaluation of individualised patient centered care, ensuring a holistic approach that is sensitive and responsive to the patients' ever-changing needs, maintaining the patients comfort, dignity and privacy at all times.
About the role: Care at Home Carer - full clean driving licence essential
You will be delivering direct care, treatment and emotional support for patients as required within set protocols/policies in accordance with the patient's individual care plan, working flexibly across all care services provided by Mountbatten.
This will involve working in patients' own homes or other places of care, and work in collaboration with colleagues to ensure the delivery of high quality, safe and compassionate care and support.
This role requires promoting and maintaining effective liaison with the care team, and other health and social agencies involved in the patients' care.
About You:
You will be qualified to Level 2 Health and Social Care Certificate and have previous care experience.
You will be required to deliver a high standard of direct patient care, with both direct and indirect supervision as delegated by the qualified nurse.
