Craneswater Group Practice is seeking to employ a Practice Nurse.
This is an exciting opportunity where no two days are the same. This role is to assist the medical team in the care of our patient population, including treatment, preventative care, screening, and patient education in the practice and local community.
As an experienced nurse, you will join our proactive team, dedicated to offering health and well-being to our patients. Your primary focus will be health promotion and health prevention.
This position's hours are negotiable with a minimum of 28 hours. This role will include extended hours with one late evening until 8 pm.
The Main Duties And Responsibilities Are:
- To provide assessment, screening, treatment services, and health education advice.
- To organise and co-ordinate the provision of nursing services for the practice liaison with other members of the nursing team and other Primary Health Care professionals
- Provide nursing treatments to patients in participation with general practitioners or independently, according to agreed guidelines and patient group directives
- Provide general and specific health screenings to the patients with referral to general practitioners as necessary both in the practice and for our patients who are either house-bound or living in Residential and Nursing Homes attached to the practice
- To assist medical personnel in the care of our patient population to include treatment, preventative care, screening and patient education in the practice and in the local community.

Craneswater Group Practice is a dedicated team of five GP partners, three salaried doctors, three nurse practitioners, plus two practice nurses and three HCAs. Overall, we have a team of 42. List size 11,100.

Major Duties And Responsibilities
Providing assessment, screening, treatment services, and health education advice.
- Organise and co-ordinate the provision of nursing services for the practice in liaison with other members of the nursing team and other Primary Health Care professionals.
- Provide nursing treatments to patients in participation with general practitioners or independently, according to agreed guidelines and patient group directives
- Provide general and specific health screenings to the patients with referral to general practitioners as necessary, both in the practice and for our patients who are either housebound or living in Residential and Nursing Homes attached to the practice
Clinical Knowledge In The Following:
- Wound care/removal of sutures and staples
- ECGs
- Venepuncture
- New patient medicals
- Immunisation routine, childhood, and travel
- Women's health cervical cytology, contraception
- Travel medicine
- Doppler and compression therapy
- Pathological specimens and investigatory procedures
- Undertake the collection of pathology specimens, bloods, swabs, smears, etc. for daily collection
- Perform any investigatory procedures requested by the doctors
Treatment Room supplies and equipment
- In participation with the Office Manager and other members of the nursing team, ensure the maintenance of equipment and stock levels relating to patient care.
- Maintain good order of the treatment room.
Pathological specimens and investigatory procedures
- Undertake the collection of pathology specimens, bloods. swabs, smears, etc., for daily collection at mid-day.
- Perform any investigatory procedures requested by the doctors.
Administrative and professional responsibilities
- To participate in the administration and professional responsibilities of the practice team.
- Ensure accurate details of all treatments (where applicable) are recorded in patients notes.
- Ensure accurate data entries are made on the computer record of any patient contact and admin issues, including consultations, telephone advice and clinical facts, e.g. blood pressures, peak flows, cervical status, vaccinations & immunisations.
- Ensure accurate completion and submission of all administrative forms which need forwarding to the Business Manager, e.g. Overtime sheets, holiday request slips, study day requests etc
- Ensure collection and maintenance of statistical information required for regular and ad hoc reports.
- Attend and participate in practice meetings as required.
- Maintain a notice board in waiting areas designated for patient health care/education as required.
- Ensure that any information relating to personal details is not discussed or divulged to any unauthorised person and is regarded as strictly confidential at all times.

- Co-operate and participate as required in any research projects within the practice.
Liaison
- Maintain effective liaison with partners, practice manager, staff and other agencies concerned with patient care and with all other disciplines within the practice, with appropriate regard to confidentiality.
Professional development
- Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development.
- Engage with the internal annual appraisal process and maintain a comprehensive PDP.
- Complete Mandatory Training on an annual basis
