Children's Community Nurse
An exciting opportunity has arisen to join the Children's Community Nursing team here at Stockport for a 9-month fixed-term or Secondment covering maternity leave.
Duties
- You will be responsible for delivering care to children with acute and chronic health needs within their own homes across 7 days.
- You should be experienced in working with children who have acute health needs and have an understanding of children with complex health care needs.
- You must be able to work flexibly between the hours of 8 am and 9 pm, 7 days a week and be able to support an overnight on-call service for children who are at the end of life.
- You will be at the forefront of assessing the child, planning, implementing care and evaluating success.
Safeguarding
Safeguarding of children is paramount, and you will be responsible for promoting this by implementing the Trust's policies and procedures and acting promptly on concerns, communicating effectively and sharing information appropriately.

About Role
- The post holder will work as an experienced and autonomous practitioner within the Children’s Community Nursing Service.
- Providing high-quality, evidence-based nursing care to children and young people with a range of acute, chronic and complex health needs.
- The role requires clinical expertise, leadership responsibilities, and contribution to service delivery, development and evaluation in line with Trust objectives and national standards.
