Children’s Continuing Care Team - Family Support Worker Band 4.
The Children’s Continuing Care Team provides specialist care to children and young people with complex health needs in community settings, 24/7, across Derby and Derbyshire. Therefore, the ability to drive/travel independently is essential.
We are looking for motivated, enthusiastic Band 4 Family Support Workers to join our team. The Band 4 position is a senior role and requires experience working with those receiving long-term ventilation and complex health needs.
These complex health needs vary and can range from epilepsy, enteral feeding, tracheostomy care, to full home ventilation, including some children with life-limiting conditions.

Essential Information
Your shift pattern will depend on the children and young people you are allocated to work with. See further details below:
- Because of the role's nature, staff must enrol in the DBS update service to ensure the safety of the patients and families we care for in the community.
- You'll work a variety of clinical shifts, including nights, weekends, bank holidays, and twilight shifts.
- We also have some day shifts and some long days.
- These shifts will be worked in the Children's Homes, Nursery/Schools, or Respite Centres, or occasionally at Royal Derby Hospital, either as a lone worker or alongside team members when 2 staff are required.
- You will need to move and handle patients weighing up to 70kg with 2 staff/parent, and wear PPE, including FFP masks, as required.
