Role Overview
We are looking for an experienced Care Manager to oversee the coordination and delivery of complex care packages, ensuring all clients receive safe, effective, and person-centred care.
This role involves managing staff allocation, maintaining compliance, and ensuring high standards of service delivery.
Key Responsibilities
- Manage and coordinate day-to-day operations of complex care packages
- Ensure all shifts are safely staffed with suitable and competent personnel
- Work closely with recruitment to ensure effective staffing pipelines
- Maintain strong relationships with:
- Clients and families
- Case managers
- Healthcare professionals
- Monitor and ensure compliance with:
- CQC standards
- Company policies
- Safeguarding procedures
- Conduct regular reviews of care packages and staff performance
Essential Requirements
- Experience in a care coordination or care management role
- Strong understanding of complex care packages
Knowledge of:
- Manual handling
- Safeguarding
- Risk assessments
- Excellent organisational and time management skills
- Ability to work in a fast-paced environment
- Strong communication and problem-solving skills
Desirable
- NVQ Level 3 or above in Health & Social Care
- Experience working within a complex care or domiciliary care agency
- Understanding of rota systems and workforce planning
