As a Support Worker, you will:
Deliver person-centred care and support in line with individual care plans, promoting dignity, independence, choice, and recovery.
Support individuals with a range of mental health conditions using positive behaviour support and de-escalation techniques where necessary.
Build positive, professional relationships with service users while providing emotional and practical support.
Providing support, companionship, communication and engagement with service users.
Medication support, assist with daily living activities, meal preparation, household tasks, and personal care where required.
Encourage participation in education, employment, volunteering, social, and community activities, and support attendance at healthcare and other appointments.
Monitor and report changes in physical or mental health, behaviour, and wellbeing, recognising early signs of deterioration and escalating concerns appropriately.
Maintain accurate, confidential care records and comply with safeguarding, health and safety, infection prevention, and organisational policies.
Work collaboratively with colleagues, families, and healthcare professionals to deliver safe, coordinated, and high-quality care.
Complete accurate care records, daily notes, and medication documentation, ensuring compliance with CQC standards, company policies, and confidentiality requirements.
Follow all safeguarding, health and safety, infection prevention and control, moving and handling, and medication management procedures to ensure safe, high-quality care.
Requirement:
- Right to work in the UK
- Experience Care Assistant/Support worker
- Flexible - Have the flexibility to work day shifts, night shifts and alternative weekends as and when required.
- Experience working with adults aged 18+ as a care assistant or support worker is required for the role
