LIFE Band 6 Physiotherapist

Hours per week: 30 hours per week, Band 6
Working as part of the LIFE therapy team, the post-holder will provide therapy assessment and treatment of patients, identifying specialist needs and will develop, implement and evaluate programmes of care.
The post-holder will work collaboratively with the team and external providers, to ensure the right care, by the right person, at the right time, and in the right place.
They will aim to enable patients to regain independence or achieve their optimum rehabilitation potential, and to prevent unnecessary hospital admission. In addition they will assist with facilitating early hospital discharge.

- Working in the Community, visiting residents of Croydon in their own homes, care homes and rehabilitation bed settings.
- The post holder will provide therapy assessment and treatment of patients, participate in coordinating the service.
- You will have responsibility of managing and supervising Support Workers and administration tasks within the LIFE therapy team.
- The post holder will participate in MDT meetings and family conferences, make independent evidence based clinical decisions to influence practice,
- Have knowledge and awareness of HR policies and ability to manage staff by utilising these policies
- Be responsible for their own professional development and clinical skills through attending courses, conferences, workshops
- Being aware of national and local policies, to develop an extensive knowledge of chronic disease processes and management.
- The post holder will be expected to work in accordance with the Trust’s policies taking responsibility for the Health, Safety and Welfare of self and others; to eliminate unlawful discrimination and promote equality and diversity in the workplace and positively promote equality of opportunity in service delivery.
- The post holder will adhere to the Trust Infection Control Policy, procedures and guidelines and ensure confidentiality at all times.
- The post holder will be required to travel independently across the borough. Therefore a UK driving licence and access to a vehicle for work purposes will be required.
