Position Overview:
We are seeking a compassionate, experienced and organised Care Coordinator to join our dynamic team.
In this role, you will be responsible for coordinating carer rotas, facilitating communication between clients, families, multidisciplinary teams, and carers to ensure that our clients receive the highest quality of care.
Key Responsibilities:
- Coordinating schedules, preparing rotas and communicating with staff, clients and their families.
- Ensuring care workers provide the quality of care that meets the standards of the company.
- Day-to-day management of care packages and working closely with team members to ensure service user and carer concerns are appropriately and effectively addressed.
- Engage in business development to foster the company’s commercial growth.
- Mentoring and supporting Care Workers.
- Stay informed about community resources and services that can benefit clients.
- Processing New client referrals.
- Monitoring of care visits, logging in and out times
- Be part of the emergency on-call.
- Complete supervisions and appraisals, helping carers grow and maintain great practice
Experience and Qualifications:
- A minimum of 2 years of proven experience as a care coordinator
- Must have a level 2 diploma in Health and Social Care
- A good understanding of CQC standards and best practice
- Excellent planning and organisational skills, along with administration experience

Pay: Up to £27,000 depending on level of experience
