Clinical Specialist Nurse – Home Infection Management Service (HIMS)
An exciting opportunity has arisen to join the Home Infection Management Service (HIMS) as a Band 6 Clinical Nurse Specialist.
The post holder will play a key role in delivering same-day and out-of-hospital intravenous (IV) therapy services for patients requiring treatment for infections and other clinical conditions. Working as part of a specialist multidisciplinary team, you will support patients to receive safe, effective care in their own homes, reducing the need for hospital admission and enabling earlier discharge where appropriate.
You will be responsible for assessing, planning and delivering care for patients receiving oral or IV antimicrobial therapy, undertaking autonomous clinical assessments and making evidence-based decisions within established service protocols. The role requires close collaboration with the Senior Band 7 HIMS Clinical Nurse Specialist, medical teams, community services and primary care colleagues to ensure seamless patient pathways and high-quality care.
A key focus of the role is to maximise opportunities for hospital avoidance and early supported discharge by identifying suitable patients in acute, outpatient and community settings. You will provide specialist advice, monitor treatment progress, support patient education, and contribute to service development and quality improvement initiatives.

Main duties of the job
The post holder will play a key role in identifying patients suitable for early discharge on oral or intravenous antibiotic therapy, supporting inpatient teams to facilitate timely discharge and ensuring safe continuation of treatment within the community. The role will involve receiving and assessing consultant referrals from outpatient clinics to establish home IV therapy pathways, helping to avoid unnecessary hospital admissions wherever appropriate.
Working as part of the Home Infection Management Service (HIMS), the post holder will undertake patient assessments, coordinate care plans, arrange treatment delivery, and monitor patients receiving antimicrobial therapy outside the acute hospital setting. The role requires close collaboration with medical staff, ward teams, community services and other healthcare professionals to ensure seamless patient care and positive outcomes.
The post will be based within the Intravenous and Vascular Access Services (IVAS) Unit at Wexham Park Hospital, with cross-site working at Frimley Park Hospital as required. The successful candidate will work closely with the wider IVAS team, contributing to all aspects of the service, including supporting vascular access provision, IV therapy management and service development to meet the needs of patients across the Trust.
