Care Coordinator
Salary: £27,905pa

Job Description:
In this role, you will play a key part in supporting our Care Workers and ensuring our service users receive consistent, high‑quality care. You will coordinate staff using our PASS system, manage rotas, and make sure all updates to care plans, medication, and visit schedules are communicated promptly and clearly. Your leadership will help our team feel supported and our service users feel safe, respected, and well cared for
Applicants should have previous experience as a Care Coordinator, or be established Field Care Supervisors seeking the next step in their development. The successful candidate will demonstrate strong teamwork, a flexible approach to service needs, and a commitment to high‑quality care.
You will need
- Applicants for this role must have previous experience as a Care Coordinator or similar role minim 1 year
- Strong IT skills with confidence using digital systems
- Effective time management and the ability to prioritise workloads
- Strong and effective communication skills, both verbal and written
- Experience managing care packages and coordinating support
- Ability to liaise professionally with Social Workers and Local Authorities
- Knowledge and understanding of person‑centred care planning
- Carry out out-of-hours duties on a rota basis (additional payment)
- Applicants must have their own transport.
