Community Nursing Sister

Please Note
You will be supporting the staff within the team, have the privilege of delivering high-quality nursing care to our patients and supporting those around them to ensure we keep them in their own homes for longer, managing either their long-term or short-term conditions.
A full UK driving license is essential for this post. Please note that an international driving license is not accepted.
Secondment will be considered for internal candidates. If you are successful after the interview, you must obtain approval from your current line manager to accept the offer.
Successful applicants will be required to provide an immunisation and vaccination report from either their current UK occupational health provider or their UK GP.
Role
- Responsible for the coordination and provision of planned and unplanned care to patients and their carers in a defined geographical area.
- Triage and accept referrals direct from other disciplines, patients and carers and refer to other agencies where appropriate.
- Case manage packages of care and work within the framework of the community nursing team by providing holistic care
- Undertake comprehensive risk assessments associated with the care of patients in order to ensure nurses’ and carers’ safety.
- Act as a role model for other staff and students, demonstrating high standards of practice and professional conduct.
